Academic Alert Evidence Waiver

Students may choose to waive the evidence-viewing step and proceed directly to submitting an academic appeal.

By submitting this form, I confirm that:

  • I have a sufficient understanding of the incident that led to the Academic Alert.
  • I understand that:
    • I have the right to view the evidence, but I am choosing to waive this step and proceed directly to submitting my academic appeal.
    • The appeal committee will review my appeal using all available evidence, even if I choose not to review the evidence in advance.
    • I understand that the decision regarding my appeal is final. If the appeal is denied, it is not subject to further appeal.

  • Enter the name of your Course Instructor *
  • Enter your Assignment Name / Exam *
  • For cases where the outcome may include suspension or expulsion, evidence viewing is strongly recommended. If you choose to waive evidence viewing in such a case, please confirm your decision by copying and pasting the statement below into your email (skip if not applicable):
    I understand this case may result in suspension or expulsion, and I choose to proceed with my appeal without reviewing the evidence.
